1.The Choice Conception Chart is provided by CHOICE BABY SDN. BHD. as master licensee and distributor subject to these terms and conditions and on the understanding that the applicant has provided correct information on this Application Form and paid the full cost for the conception chart.

2.Key questions on the Application Form are marked with an asterix and an incorrect or false answer to these questions (if require any amendments) will incur additional payment for another conception chart. It is the responsibility of the applicant to ensure that her personal information (name, date of birth, blood group etc.) and answers to the key questions in the Application Form are correct.

3.The CHOICE Conception Chart is strictly for pre-conception gender selection. It is not a medical tool, nor does it provide any medical advice for fertility, avoidance of sex-linked diseases or any other illnesses.

4.The CHOICE Conception Chart only shows the favourable days to attempt conception naturally for gender selection. It has no link of any kind to factors spontaneous abortion, miscarriages, pregnancy loss, premature birth, birth defects, congenital abnormality, chromosomal aberrations in the fetus or in the neonate, or any other abnormality and the company and its licensees do not accept any responsibility for these conditions and for conceptions and pregnancies with such outcomes.

5.The CHOICE Conception Chart is not a fertility chart as the polarity cycle on which it is based is independent and separate from the ovulation/menstrual cycle. It cannot be used for fertility purposes.

6.The company provides a 100% money back guarantee to any mother who delivers the opposite gender to the one requested for in this form. The applicant is required to submit a certified true copy of the child’s Birth Certificate, certified by either Notary Public or lawyer within 60 days of the birth of the child to the company.

7.The company shall refund the full amount received from the applicant for the conception chart within 60 days from receipt of the certified true copy of the child’s birth certificate showing the opposite gender to the one applied for the shown on the CHOICE Conception Chart. The fee for the CHOICE Conception Charts is non refundable except in the case of the full refund as herein stated

8.The company and the applicant agree that in the event the conception results in an opposite gender from the one applied for, the only indemnify and liability is the payment of full refund as stated in the item [ 6 and 7] , and the applicant shall indemnify the Company and its Licensees from any legal action by third parties.

9.A Valid contract comes into existence between the Applicant and the Company upon receipt of payment and acceptance of this Application Form by the company

10.The parties hereby agree to adhere to all terms and conditions of this Agreement.
